Values & Principles

As Health and Social Care professionals we are committed to:

Care, Compassion and Respect

Quality and Excellence
We are commited to supporting services to deliver care of the highest quality. We will constantly strive for excellence and are committed to making continual evidence based improvements.

Competence and Knowledge
We are commited to ensuring organisations, teams and individuals have the knowledge, skills and tools to respond to service needs and contribute to improving the health and well being of the people we serve, their families and the communities in which they live.

Partnership and Collaboration
We recognise that delivery of health and social care services is complex and relies on the connection and collaboration of many. We are committed to working in partnership and sharing responsibility with our colleagues, service users, communities, and other organisations to integrate services and improve the health and well being of the population they are a part of.

Integrity, Accountability and Ethics
We are committed to fulfilling our duties and obligations and honouring our commitments to each other and all we serve. We communicate with openness, transparency and honesty. We hold ourselves to the highest ethical standards in our work and take responsibility for the consequences of our actions.

Creativity and Innovation
Our services are personalised to the needs of the team and those who use the services we support. We respond in a solution focussed manner, using a living theory approach to learning and creatively seek new ways of responding to old problems and familiar challenges. We support advancements in health and social care service provision by seeking, facilitating and promoting learning, research and education.

Pride and Recognition
We value everyone’s contribution and commitment to excellence. We demonstrate pride in self and others valuing the gifts and talents of our team and those we work with.

We support services to respond to the unique needs of individuals and communities by recognising difference, valuing and accommodating the different needs of people living in diverse populations.

Person Centred and Integration
We strive to support the provision of a personalised, seamless system of service delivery that is easily navigated and clearly understood. We will provide service users with information that explains who is working with them, what their role is, how care and support will be provided, where and when this will occur. We believe all service users and their families/carers should have a shared contract of support that explains how agreed outcomes will be achieved and where appropriate where professional and service interventions will cease.

We are aware of the importance of balancing the services we support with the physical and financial resources available, in order to ensure a sustainable structure and framework to support service delivery. We will support investment in what is effective in meeting needs, and help identify how this investment is financed to ensure that services are supported, led and managed efficiently.

Team - Purpose
The team’s purpose is to support professionals and practitioners to engage in safe, ethical and competent practice. The role of the team is to ensure that health and social care professionals and their practice contribute to the delivery of quality person centred outcomes. Interventions are designed to support safe, competent & ethical practice. Central to this is facilitating health and social care professionals’ participation in practice and administrative policy decision making. Using Action Learning methodology ensures the cognitive whole of the organisation or staff team and the people who use the service is utilised to inform service development, design / redesign while learning is embedded in the culture of the organisation.
Using a Living Theory approach underpinned by person centred thinking informs service development by embedding a shared language through dialogic learning. This approach supports the use of evidenced based practice to continually improve and drive up standards. We recognise that teams are made up of individuals who have their own unique characteristics. Harnessing what people do best, in a system that manages around their weaknesses, enables individuals and teams to develop within an appreciative and supportive environment.
Leaders in the organisation understand what motivates and drives individuals to be successful and focus on these strengths to create a shared dialogue from which the team flourish.
The teams philosophy is underpinned by the values of person centred care and support which, through a shared contract (personalised care plan) maintains the people and families served by Planning with People at the heart of ethical practice. This is reflected through the principles of integrity, responsibility, accountability, confidentiality and advocacy. Interventions provided by the team and those they support are communiecated through the values of: commitment, truthfulness, transparency, integrity, social justice, clarity of expectations, passion and humour.
